Job summary

CAMBA in Bronx, NY seeks a full-time Case Manager to support families through housing and social services programs. You will perform intake, assess needs, create and maintain client files, and connect clients with community resources to promote self-sufficiency.

Responsibilities include coordinating referrals, scheduling appointments, and documenting progress in CARES. You will advocate for clients and collaborate with diverse partners to ensure holistic support.

Qualifications

We take a comprehensive approach by offering more than 160 integrated programs in: Education & Youth Development, Family Support, Job Training & Employment Support Services, Health, Housing, and Legal Services. We reach more than 65,000 individuals and families each year at our 90 locations, including 10,000 youth. CAMBA serves a diverse cross section of New Yorkers from new mothers in Brownsville to job seekers in the Rockaways. More than half of our clients are immigrants and refugees from around the globe. Over 85% of our families are in living in poverty, reflecting the challenges faced by nearly 1.7 million New Yorkers today.

CAMBA operates six transitional housing facilities for families with children experiencing homelessness funded by the New York City Department of Homeless Services. Each facility offers clients a supportive, structured, therapeutic, safe, and drug-free environment. We provide comprehensive case management and housing placement services to ensure that families move to permanent housing as quickly as possible. Recreation and evidence-based programming are also available to enhance the services to families, school-aged children, as well as parenting support and support to young adults.

Position: Case Manager
Reports To: Case Manager Supervisor or Director of Social Services
Location: Bronx, NY 10468
What The Case Manager Does:
  • Maintain professional relationships with clients and client confidentiality.
  • Practice Universal Precautions/Standard Protocol & Procedures.
  • Comply with all Federal, State, City and CAMBA security and privacy policies intended to protect the security and privacy of individually identifiable health information.
  • Establish and maintain professional relationships with clients and client confidentiality to engage them in case management services.
  • Review all documentation establishing clients' eligibility for program and make file copies.
  • Create and maintain client files.
  • Input client data and client progress information into CARES database.
  • Conduct initial intake and assessment of clients and families' needs and periodic re-assessments.
  • In collaboration with clients, prepare initial and periodic revisions of independent living plans including short-term and long-term client goals.
  • Assist clients in attaining their goals by identifying and locating community resources for clients.
  • Refer clients to permanent housing opportunities and appropriate services both within and outside CAMBA.
  • Schedule appointments for clients with referral organizations.
  • Escort clients to appointments (i.e. housing, entitlements, educational, medical, social service, etc.).
  • Assist clients in completing applications for benefits and entitlements, and/or process applications on clients' behalf.
  • Work with clients to break through barriers to client goals and to assist clients in advocating themselves and in moving toward self-sufficiency.
  • Recommend and implement strategies to persuade clients to participate more fully in this process.
  • Monitor clients' progress toward their goals (dates achieved) via regularly scheduled contact and/or face-to-face unit and office visits, and document via progress notes.
  • Follow up with clients and referral organizations regarding client contact and progress with referral organization.
  • Act as client liaison/client advocate with outside organizations regarding such matters as education, healthcare, housing, legal issues, etc.
  • Provide all required information for weekly/monthly/quarterly/annual reports.
  • May follow up with clients for a period of time after successful completion of their primary goals to ensure client stability.
  • May complete referral for aftercare services.
  • May prepare marketing materials for the program.
  • Tasks may be modified, expanded and/or assigned over time.
Education, Licenses and/or Certifications Required:
  • High School Diploma or equivalent and three (3) years of applicable experience or associate degree and two (2) years of applicable experience or bachelor's degree and one (1) year of applicable experience.
  • Applicants currently enrolled in a program and on track to complete the minimum required education, license, and/or certification within the next three months, and who meet the minimum experience requirements, are encouraged to apply. However, employment is contingent upon satisfying requirements for the position.
Minimum Experience and Skill Required
  • Experience required listed above with minimum education
  • Computer literacy in MS Office and other basic office technology
Preferred Qualifications:
  • Bilingual (English & any other language) preferred.
  • Working knowledge of CARES
Pre and/or Post Employment Requirements:
  • Must obtain State Central Registry (SCR) clearance, The Register of Substantiated Category One Cases of Abuse or Neglect aka Staff Exclusion List (SEL) clearance and Criminal history information checks via fingerprinting with New York State Division of Criminal Justice Services (DCJS) prior to start of employment.
  • Ability to maintain clearances throughout the duration of employment.
  • Required to become First Aid/CPR certified.
  • Required to become certified in overdose prevention.
  • Submit medical Tuberculosis (TB) test results as part of the onboarding process.
Compensation

:$46,144 - 50,393 annually
