Responsibilities

  • Perform case management services with in-house treatment services and external supportive services.
  • Maintain calm, professional demeanor during crises or emergencies.
  • Schedule appointments for clients with outside agencies.
  • Assist clients with benefits acquisition (SSI, public assistance, food stamps).
  • Complete required interagency and other necessary paperwork.
  • Assist with unit group activities.
  • Assist clients with prevocational, vocational, or educational programs; housing issues; medical services.
  • Conduct toxicology testing as instructed by the program manager.
  • Recruit eligible, underserved consumer groups through outreach as needed.
  • Provide services in settings accessible to clients and follow up to ensure services were delivered.
  • Transport clients in Center or own vehicle as required.
  • Complete daily activity logs and progress notes; ensure confidentiality and proper documentation.
  • Develop and implement a plan of care with consumer input and signed treatment plan.
  • Monitor confidentiality in accordance with policy and legal requirements.
  • Complete required reports and evaluations.

Job Summary

This position is eligible for $2,500 retention bonus.

The Providence Center Case Manager Crisis Stabilization Unit (CSU) performs a variety of services for clients: screenings, intakes, assessments, orientation, counseling, crisis intervention, and referrals.

Duties & Responsibilities
  • Perform case management services with in-house treatment services and external supportive services.
  • Perform in a calm and professional manner during crisis or emergency situations.
  • Schedule appointments for clients with outside agencies.
  • Assist clients with benefits acquisition (social security supplemental insurance (SSI), general public assistance, food stamps, advocates office, medical doctor).
  • Complete required interagency and other necessary paperwork.A

  • Conduct toxicology testing for all clients as instructed by program manager.
  • Recruit eligible, underserved consumer groups through outreach as needed.
  • Provide services in a setting most acceptable and accessible for individual consumers. Provide follow up to ensure appropriate services have been delivered.
  • Transport clients in own vehicle or TPC vehicle to planned activities and appointments.
  • Drive carefully and abide by traffic rules.
  • Ensure that seat belts are worn by all traveling in Center owned or employee owned vehicles.
  • Complete recordkeeping in accordance with Health Information Services (HIS) and other Center requirements within required time frames and abide by Center confidentiality policy.
  • Complete daily activity logs.
  • Complete progress notes with appropriate format.
  • Develop and implement a plan of care with ongoing consumer input.
  • Complete a clear, specific, behaviorally oriented treatment plan as required, with consumer signature.
  • Monitor confidentiality in accordance with Center policy and legal requirements.
  • Complete required reports and evaluations (MHPRR checklist, Psycho-social form).
  • Follow required office procedures in a cooperative manner.
  • Initiate and process incident reports as per Center guidelines.
  • Adhere to Infection Control protocols.
  • Maintain cooperative relationships with Center staff, clients, community agencies and the public.
  • Communicate with various local agencies and other departments or teams at The Center.
  • Work in cooperation with co-workers and supervisory staff.
  • Deal with team members regarding problems in a constructive manner.
  • Accept and use direction and supervision.
  • Benefit from constructive criticism.
  • Attend trainings, case presentations and conferences.
  • Provide feedback to staff on the content of educational programs attended.
  • Attend mandatory in-service trainings and other trainings required.
  • Work flexible but scheduled hours (possibly including some weekend and evening hours).
  • Serve on TPC committees.
  • Perform other duties as assigned to support client goals and program needs.
Requirements
  • Associate's Degree Required; Bachelor's Degree Preferred; Field of Study: Human Services or related.
  • Experience in Human Services field preferred.
  • Must have valid driver's license, registration, current inspection sticker and insured auto.
  • This position may be based at various Center locations, community or client home settings.
  • Strong knowledge of community resources and systems, including housing, education, employment, and healthcare.
  • Excellent organizational, problem-solving, and time management skills.
  • Ability to build rapport and establish trust with diverse clients and community partners.
  • Cultural competence and commitment to providing equitable and inclusive services.
  • Ability to communicate effectively, both verbally and in writing, to convey information clearly and collaborate with multidisciplinary teams.
  • Proficiency in documentation and electronic record-keeping systems, including ability to document in electronic medical records.
  • Ability to travel within the community with reliable transportation.
  • Bilingual preferred. For positions designated as bilingual (Bil), proficiency in the required language is mandatory, and the language assessment must be passed no later than 30 days of starting the role.
