Position Overview

Community-Based Case Manager - Morrow and Surrounding Counties

CareStar, Inc. is seeking a Community-Based Case Manager in the Clinical Department to help individuals remain in the least restrictive environment, maintaining independence and dignity. The role involves coordinating services to ensure clients receive appropriate, planned, and effective care, while respecting their preferences and self-determination.

Responsibilities

• Active license as a Licensed Social Worker, Licensed Independent Social Worker, or Registered Nurse.
• At least 12 months experience in home and community-based services within the past 5 years.
• Knowledge of Medicare, Medicaid, home health care, and Ohio Home Care waiver programs is desirable.
• Approve funds for services, develop annual budgets, and ensure standards are met.
• Stay current through continuing education and utilize knowledge of relevant programs and laws.
• Lead the development and implementation of care plans, including service authorization.
• Conduct home visits and monitor services in compliance with company policies and program rules.
• Perform Person-Centered Planning, assessments, and annual Level of Care reviews, considering cultural factors.
• Complete incident reports, identify high-risk situations, and take appropriate actions.
• Adhere to company policies, maintain confidentiality, and follow information system policies.
