Job summary

A community service organization in Blacksburg, Virginia is seeking a Case Manager to provide vital case management services for individuals with mental illness and substance use disorders. This role requires collaboration with the Jail Diversion Team to ensure clients receive appropriate services and support. Candidates must have at least a High School Diploma and experience in social services. Additional compensation will be considered for multilingual candidates. Competitive starting hourly rate is $18.75 depending on experience.

Qualifications

  • High School Diploma is required with at least one year of experience in a setting working with individuals with substance use disorders and/or possible co-occurring mental illness.
  • Bachelor's Degree in Education, Social Work, Psychology, or Human Services related field is preferred.
  • Registration as a QMHP or trainee is preferred.
  • CSAC is preferred.

Job description

New River Valley Community Services Bridge/Jail Diversion are seeking a Case Manager! The Case Manager provides case management services for individuals with mental illness, substance use, or co-occurring disorders who demonstrate a need for active case management services. Works as part of the Jail Diversion Team to coordinate services for clients enrolled in the program and to ensure compliance with program requirements/expectations. Also provides discharge services and active follow-up to engage the client in appropriate services. The Case Manager coordinates services and/or resources needed by the client to support their basic needs and to enable them to remain and function optimally in the community. Attends treatment team meetings, specialty docket hearings, and maintains contact with referral sources (e.g., Probation, DSS, etc.) as needed.

Starting Hourly Rate: $18.75-Depending on Experience

Education Requirements
  • High School Diploma is required with at least one year of experience in a setting working with individuals with substance use disorders and/or possible co-occurring mental illness.
