Job summary

Acenda Health in New Jersey is seeking an experienced Care Manager on an hourly basis to coordinate client care, connect them to services, and support improved quality of life. You will work within Integrated Case Management Services - Gloucester (ICMSG) to develop personalised plans and monitor progress.

You will collaborate with physicians, social workers, and families; ensure timely access to resources; maintain compliant documentation; and adapt to client needs with a flexible schedule.

Qualifications

• Bachelor's/Master's degree in psychology, social work or related field, plus 2 years of relevant experience.
• Must have a valid driver's license with an acceptable driving record.
• Ability to analyze work, set goals, develop plans and utilize time effectively.

Responsibilities

• Care Coordination: Oversee and manage client care plans, ensuring timely access to services and resources.
• Collaboration: Work closely with providers, clients, and families to develop and implement comprehensive care plans tailored to individual needs.
• Monitoring Progress: Regularly assess and monitor client progress, addressing any barriers to treatment and adjusting care plans as necessary.

Duties and Responsibilities

• Care Coordination: Oversee and manage client care plans, ensuring timely access to services and resources.
• Collaboration: Work closely with providers, clients, and families, to develop and implement comprehensive care plans tailored to individual needs.

• Adheres to a flexible schedule based on client and program needs.
• Demonstrates competent written and verbal communication with referral sources and external customers.

• Works collaboratively with coworkers and agency staff to effectively communicate and problem-solve.
• Meets obligations of the program (contractual, licensing, Medicaid, etc.).
• Follows program policies and procedures and encourages the same in others.
• Educates self on issues impacting the program population.
• Operates conscientiously in accordance with program budget (i.e., use of client engaging funds, office supplies).

Physical Requirements
