Qualifications

• Bachelor’s degree in social work, psychology, or related field, or an associate degree with 2+ years CM experience.
• Must be over 21 years of age.

Responsibilities

• Accomplishes clients’ care by assessing treatment needs; monitoring and evaluating treatment plans and progress.
• Determines clients’ requirements by completing intake interviews and determining need for evaluations; reviewing therapist evaluations, objectives and plans.
• Scheduling clients with Addiction Psychiatrist if needed.
• Monthly scheduled appointments with clients according to appointment schedule.
• Establishes treatment programs by setting schedules and routines; coordinating services; arranging resources.
• Monitors cases by verifying attendance; observing treatments and progress; advocating for needed services; intervening in crises; providing support.
• Maintains clients’ records by reviewing case notes; logging events and progress.
• Communicates clients’ progress by conducting monthly coordination meetings with client’s clinician team.
• Prepares clients’ discharge by reviewing and amplifying discharge plans; coordinating discharge and post-discharge requirements; providing resources.
