Job summary

  • License: Current unrestricted Mississippi State licensure as a nurse.
  • Certification: Must complete and maintain BCLS certification and TB certification as applicable; de‑escalation training within 90 days.
  • Experience: Two (2) years’ nursing experience in ambulatory or acute care relevant to case management.

Responsibilities

  • Daily discharge planning meetings and coordination between inpatient and outpatient care management.
  • Manage resources to improve clinical quality and financial outcomes; ensure communication across teams.

Job description

Position Overview

Case Manager focuses on clinical quality and resource management resulting in defined clinical satisfaction and financial outcomes. They coordinate patient care to a population of different ages within the Singing River Health System. All performed duties must be in accordance with Singing River Health System procedures and policies, accreditation organization, and governing guidance and publications for health care employees.

Case Manager must be able to work effectively under pressure, multitask, be self‑directed, and have strong written and oral communication, analytical and critical thinking, conflict management, team‑building, and organizational skills. The ability to perform numerous tasks simultaneously with minimal errors is required.

Responsibilities

Daily discharge planning meetings: addressing barriers that may inhibit discharge and targeting successful transitions from one level of care to the next. Create a seamless line of communication between the inpatient care management team and outpatient case management team.

Qualifications

Education Graduate of an accredited School of Nursing; Bachelor’s Degree in Nursing preferred.

License Current unrestricted Mississippi State licensure as a nurse.

Certification Must complete and maintain BCLS certification upon hire and/or transfer. Required to be current and maintain TB certification as applicable to their position and scope of practice. Must have de‑escalation training completed by the end of position orientation (90 days).

Experience Two (2) years’ nursing experience in ambulatory or acute care that has relevance to the practice of Case Management.

Reports To Director- Patient Resource Management

Supervises None.

Physical Demands

Work is moderately active: involves sitting with frequent requirements to move about the office, move about the facility, and travel to another facility within the SRHS service area. Work involves negligible force to lift, carry, push, pull, or otherwise move objects, including the human body. Repetitive motions of wrists, hands, and fingers while operating standard office equipment such as computer keyboard. Exposure to body fluids, sputum, and tissues that may carry infectious disease. Must be able to work schedules assigned with understanding that changes may be instituted according to hospital needs for off days, shifts, or weekends.

Mental Demands

Must demonstrate keen mental faculties, assessment and decision‑making abilities. Superior communication and speaking/enunciation skills to receive and give information in person and by telephone. Strong written and verbal communication skills. Emotional stability conducive to dealing with high stress levels. Ability to work under pressure and meet deadlines. Attention to detail and ability to multi‑task in complex situations required. Maintain collaborative and respectful working relationships throughout SRHS and other organizations.

Special Demands

Must possess superior customer service skills and professional etiquette. Proficient knowledge and ability to use a computer (must be keyboard proficient) and other office technology (telephone, fax, etc.), MS Outlook and Word. Demonstrate appropriate clinical judgment and apply professional skills to a patient population of all ages.
