Job summary

UF Health Central Florida is seeking a Case Manager to assess patient and family scenarios for clinical appropriateness and to establish plans for effective management of the continuum of care. Collaboration with the care team is essential to optimize quality of care, utilization, cost and patient satisfaction.

The role requires a Florida RN license, a minimum of five years in case management, and strong communication skills. CCM certification is preferred.

Qualifications

• Minimum of 5 years of experience in Case Management.
• CCM certification preferred.
• Must be able to read, write, speak and understand English.

Responsibilities

• Assess patient and family scenarios for clinical appropriateness and plan management of the continuum of care.
• Collaborate with physicians, nurses and social work to coordinate services and optimize quality of care, utilization and patient satisfaction.
• Perform assessment, planning, intervention and reassessment within a multidisciplinary team.
