Job summary

Columbus House, Inc. in Connecticut is seeking a Case Manager Aide to support the Permanent Supportive Housing program. The role involves welcoming clients, coordinating meetings, maintaining HMIS data, scheduling, and coordinating supplies.

You will work 40 hours per week, Monday–Friday, 9:00am–5:00pm, with responsibilities including intake, transportation as needed, and liaising with program staff. Strong organization and communication are essential.

Qualifications

  • Excellent organizational and time management skills with a deadline-oriented mindset.
  • Strong written and oral communication skills employing the English language.
  • Knowledge of and ability to effectively navigate substance abuse, mental health, and social services networks.

Responsibilities

  • Supervise access into 610 Annex building via calls from outdoor electronic callbox and/or manually answering door when necessary to clients and guests.
  • Facilitate and maintain a positive, peaceful and welcoming atmosphere as the first point of contact.
  • Provide customer service to clients, visitors, case managers and program managers.
  • Notify appropriate staff when clients appear for scheduled meetings.
  • Inform clients of staff absences and take messages for absent staff.
  • Reschedule appointments for clients when necessary.
  • Attend agency and program meetings as scheduled.
  • Order supplies monthly for all 610 Annex programs.
  • Conduct monthly satisfaction surveys with program participants.
  • Ability to communicate efficiently and effectively with individuals served.
  • Able to advocate for clients as well as support self-advocacy.
  • Must be able to set and maintain boundaries and adhere to confidentiality.
  • May provide transportation to appointments for clients at the discretion of the case manager on a case-by-case basis.
  • Ability to understand and accept different backgrounds while providing a non-judgmental approach to rendering services.
  • Complete intakes, paperwork, and input HMIS data for clients as determined by program needs.
  • Provide temporary caseload coverage for vacant positions as determined by program needs.
  • Assist Director of Housing with assigned tasks.
  • Other related duties as assigned.

Case Manager Aide

Purpose

Under the purview of the Permanent Supportive Housing Department, the Case Manager Aide seeks to create and maintain a positive, welcoming atmosphere by providing safety, organization, and program management support to clients within the Department. The Case Manager Aide is the first point of contact for all visitors of 610 Annex programs including Permanent Supportive Housing and Rapid Rehousing and performs various office functions in support of program staff.

Qualifications

  • Proficient in Microsoft Office and other common desktop and online applications
  • Knowledge of and ability to effectively navigate substance abuse, mental health, and social services networks
  • Familiar with criminal justice barriers and able to engage, motivate, and support the individuals serviced
  • Familiar with mental health, substance abuse and dual diagnosis issues and group dynamics
  • Able to work with multiple populations including youth (18-24 years old), adults, and families
  • Able to meet all documentation requirements within set timeframes
  • Knowledgeable of crisis intervention, boundaries, confidentiality, universal precautions, risk reduction, and motivational interviewing
  • Empathetic and compassionate with individuals that have experienced homelessness and factors that lead to homelessness
  • Valid drivers’ license with a good driving history

Supervised by: Single Site PSH Manager

Schedule: 40 hours per week; Monday- Friday 9:00am- 5:00pm

Salary range:$19.00
