Qualifications

  • Minimum bachelor’s degree in social work, human services, or related field.
  • 2 years prior experience in supporting father and father figures with family support/case management services preferred.
  • Case Management Credential preferred but not required.

Responsibilities

  • Research and deliver case management services within the FRANC’s program framework.
  • Coordinate and collect required assessments.
  • Complete an initial client process, screening and/or assessment within the specified program timeframes.
  • Link fathers to services and natural support systems.
  • Monitor ongoing progress and needs within the fathers; serve as liaison to connect dads to involved agencies.
  • Coordinate existing and find new services.
  • Cultivate and manage collaborative relationships with the early childhood, child welfare, behavioral health, legal, housing, and family serving community.
  • Timely data entry into participant records and data collection systems.
  • Prepare for and participate in case reviews with the Interdisciplinary Team.

## Description**PROGRAM/DEPARTMENT:** Father Resources and Network Community (FRANC) **TITLE:**Family Champion (Case Manager) **CLASSIFICATION:** Full-Time (37.5 hours per week), Non-Exempt, Hourly: $23.08 ($45,000/yr)**Position Summary:**The Family Champion (Case Manager) is a full-time direct services position responsible for connecting with fathers, creating, and managing a family support plan where concrete supports are provided by utilizing a broad range of community services and natural support systems. This position reports to the Lead Family Champion. Specific responsibilities include, but are not limited to: **ESSENTIAL DUTIES:**1. Research and deliver case management services within the FRANC’s program framework when needed. 1. Coordinate and collect required assessments. 2. Complete an initial client process, screening and/or assessment, within the specified program timeframes and guidelines. 3. Link fathers to services and natural support systems. 4. Monitor ongoing progress and needs within the fathers. Serve as liaison to connect the dads to involved agencies that can provide support. 5. Coordinate existing and find new services.2. Cultivate and manage ongoing collaborative relationships with the early childhood, child welfare, behavioral health, legal, housing, and family serving community.3. Timely data entry into participant records and data collection systems.4. Prepare for and participate in case reviews with the Interdisciplinary Team.5. Adheres strictly to policies which ensure participant confidentiality.6. Participates in and supports the agency’s performance and quality improvement processes.7. Participates in outreach development and delivery of community events.8. Participates in related training and workshops.9. Performs other related duties as assigned or requested.## Qualifications**REQUIREMENTS:**1. Minimum bachelor’s degree in social work, human services, or related field.2. Preference will be given to individuals with 2 years prior experience in supporting father and father figures with family support/case management services.3. Case Management Credential preferred but not required.4. Manage a caseload; organize and prioritize work assignments; make decisions and independent judgments; determine the appropriate course of action in emergency or stressful situations; and recognize reportable events.5. Bi-lingual (English/Spanish) preferred.6. Computer skills with proficiency in Microsoft Office (Excel, Word, Outlook), Microsoft 365 web-based applications and using internet search engines and other online research tools.7. Ability to work in the office and in the field. (Please be advised: Position is NOT a hybrid/virtual role).8. Available to work some evenings and Saturdays and schedule visits according to participants needs.9. Professional, self-directed, high-energy can-do attitude, follow-through on projects and prompt responsiveness to internal and external stakeholders.10. Good interpersonal skills, team-oriented, positive customer service focused enjoys working with others.11. Light physical demands: Regularly required to sit; use hands to finger, handle or feel; reach with hands and arms and talk and hear. Ability to walk, stand, climb stairs, kneel, bend, reach, and manipulate objects; lift and move materials up to 10 pounds on a regular basis; infrequently require to lift or move materials weighing up to 20 pounds.12. Able to travel in the communities served; must have a reliable car, active auto insurance, and valid Florida driver’s license.13. Must be sensitive to the cultural and socioeconomic characteristics of the population that Champions for Children serves.14.
