Westchester General Hospital is seeking a qualified professional to assess psycho-social needs of patients and initiate discharge planning. You will counsel patients and families and coordinate resources for post-discharge care.
The role involves group and individual therapy, collaboration with a multidisciplinary team, and ensuring compliance with mental health regulations. Associate's degree in nursing preferred; two years inpatient hospital experience with CM/UR/DCP preferred.
Assess psycho-social needs of patients assigned and initiates necessary arrangements for needed resources and/or discharge plans. Provides counseling to patients and families for care required by patients after discharge. Performs assessment/evaluation of treatment needs. Conducts group therapy sessions and/or individual therapy.
Sedentary Work:Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently to lift, carry, push, or pull, or otherwise move objects, including the human body.
Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time.

Associate’s degree in nursing, bachelor’s in nursing preferred
A minimum of two years of inpatient hospital experience with CM/UR/DCP experience preferred
