Job summary

LHH is seeking a dedicated case manager in Stockton, CA to support housing and essential services clients. The role involves direct client engagement, case management with a monthly caseload of about 40, and collaboration through weekly leadership meetings.

Onsite position with standard business hours, onsite campus access, and structured cross-training during initial weeks. Fluency in Spanish is highly preferred to serve diverse clients.

Qualifications

• Prior housing or homelessness services experience preferred.
• Experience managing caseloads and working directly with clients.
• Spanish-speaking candidates are highly preferred.

Responsibilities

• Work directly with clients to address housing needs and service coordination.
• Manage a caseload of approximately 40 cases per month.
• Attend weekly meetings with the Director of Essential Services and team members.

Job description

• Caseload: Approximately 40 cases per month.
• Training: 3–4 weeks of cross-training and shadowing with existing Case Managers.
• Systems Used: HMIS
• Spruce Health (EHR/Salesforce platform)
• Microsoft Office Suite
• Internal client-tracking system
• Weekly meetings held with the Director of Essential Services and team members.
• Approximately 135 employees onsite.
• Monday–Friday
• 8:30 AM–5:00 PM
• Gated campus with onsite parking available
