Job summary

Encompass Health Corporation is seeking a Case Manager to coordinate patient care from admission to post-discharge and act as a vital link between exceptional care and patient outcomes. You will work with an interdisciplinary team to establish care plans, monitor discharge readiness, and ensure timely coordination of services across the patient journey.

The role emphasizes clinical expertise in rehabilitation, regulatory awareness, and collaborative care planning in a national inpatient setting.

Qualifications

  • Must be qualified to independently complete an assessment within the scope of practice of one's discipline.
  • Active license in state.
  • CCM or ACM certification required or must be obtained within two years of entry into the position.

Responsibilities

  • Work with interdisciplinary team, guiding treatment plans based on patient needs and preferences.
  • Coordinate with interdisciplinary team to establish tentative discharge plan and contingency plans.
  • Participate in planning for and the execution of patient discharge experience.
  • Monitor patient experience: quality/timeliness/service appropriateness/payors/expectations.
  • Facilitate team conferences weekly and coordinate all treatment plan modifications.
  • Complete case management addendums and all required documentation.
  • Maintain knowledge of regulations/policies and department operations.
  • Review and analyze case management reports, including Key Care Indicators, and plan actions.

  • Meetwithpatient/familyperPatientArrivalandInitialVisitStandardwithin24hrs.of admission.
  • Performassessmentofgoalsandcompletecasemanagementaddendumwithin48hoursof admission.
  • Educatepatient/familyonrehabilitationandCaseManagerrole;establishcommunication plan.
  • Scheduleandfacilitatefamilyconferencesas needed.
  • Assistpatientwithtimelyprocuring/planningofresourcestoavoiddischargedelaysor issues.
  • Monitorcompliancewithregulationsfororthoticsandprostheticsorderingand payment.
  • Makeappropriate/timelyreferrals,includingdocumentationtopostdischarge providers/physician
  • Ensureaccuracyofdischargeandpayor-relatedinformationinthepatient record
  • Participateinutilizationreviewprocess:datacollection,trendreview,andresolution actions.
  • Participateincasemanagementon-callscheduleas needed.
Qualifications
  • Licenseor Certification:
    • Must be qualified to independently complete an assessment within the scope of practiceofhis/herdiscipline(forexample,RN,SW,OT,PT,ST,andRehabilitation Counseling).
    • Iflicensureisrequiredforone'sdisciplinewithinthestate,individualmustholdan active license.
    • MustmeeteligibilityrequirementsforCCMorACMcertificationuponentryintothis position OR within two years of entry into the position.
    • CCMorACMcertificationrequiredORmustbeobtainedwithintwoyearsofbeing placed in the Case Manager II position.
  • Minimum Qualifications:
    • For Nursing, must possess minimum of an Associate Degree in Nursing, RN licensure with BSN preferred. A diploma is acceptable only in those states whose minimum requirementforlicensureorcertificationisadiplomaratherthanan a associate d egree.
    • Forallothereligiblelicensedorcertifiedhealthcareprofessionals,mustpossessa minimum of a bachelor's degree and graduate degree is preferred.
    • 2yearsofrehabilitationexperience preferred.
