Job summary

Jobtailor in Louisiana seeks a patient care coordinator to align interdisciplinary teams and optimize inpatient flow. You will review admission records, determine the appropriate level of care, and identify discharge needs to ensure efficient use of hospital resources.

Work with nurses, therapists and social workers to coordinate discharge plans and achieve strong financial and quality outcomes. A current BLS certification is required and 3–5 years of inpatient experience in acute, IRF or LTACH

Qualifications

• Licensure in the state where the hospital resides preferred
• Licensed multidisciplinary professionals may include OT, PT, RN, RT, ST or SW
• Current BLS certification required
• Three to five years of inpatient experience, preferably in acute, IRF, or LTACH settings
