Job summary

NKC Health in Missouri is seeking an experienced Registered Nurse to provide clinically based case management under the supervision of Transition of Care. You will collaborate with case management and multidisciplinary teams to coordinate resources and ensure efficient documentation and reimbursement.

The role requires 2-3 years in case management, MO RN license, and ACM certification preferred within 1 year of hire.

Qualifications
