Job summary

Responsibilities

  • Provide case management services and maintain ongoing communication with youth, families, agency representatives, and members of the treatment team.
  • Coordinate or arrange required mental health and substance use screenings and assessments.
  • Complete performance summaries, treatment plan reviews, transition plans, aftercare plans, and discharge summaries; maintain accurate case records.
  • Serve as a liaison among the program, the Tennessee Department of Children’s Services, outside agencies, and each youth’s family.
  • Prepare youth and families for program visits, family conferences, transition planning, and aftercare responsibilities.
  • Connect youth and family members with appropriate community resources.
  • Assist with post-release and follow-up data collection as required by policy and contractual obligations.
  • Participate in treatment team meetings and communicate progress, needs, and placement recommendations.
  • Collaborate with leadership on performance-improvement activities.
  • Conduct periodic case-record reviews to support policy compliance.

About Youth Opportunity & Harpeth Primary Assessment Center

Youth Opportunity is a nationally recognized leader in residential and behavioral health services for youth. Harpeth Primary Assessment Center in Nashville, Tennessee, is a secure, 24-bed assessment center serving girls ages 10 to 17.
The center supports the Tennessee Department of Children’s Services by providing temporary, trauma-informed care, observation, and comprehensive evaluation services. These assessments help identify each youth’s strengths, treatment needs, and most appropriate next placement.
Youth receive structured support in a safe and compassionate environment while participating in mental health and substance use assessments, educational services, individualized programming, and transition planning. The program’s goal is to develop informed recommendations that promote healing, stability, and long-term success.

Position Summary – Case Manager Job in Nashville, TN

We are hiring a Case Manager to coordinate assessment services, documentation, communication, and transition planning at Harpeth Primary Assessment Center. Working under the clinical supervision of a licensed mental health professional, you will serve as an important liaison among youth, families, the Tennessee Department of Children’s Services, the treatment team, and other community partners.

Case Manager Responsibilities

  • Collaborate with leadership on performance-improvement activities
  • Conduct periodic case-record reviews to support compliance with Youth Opportunity policies and agency requirements
Other Duties Include
  • Maintain a high standard of role-model behavior for youth
  • Participate in required training and professional development activities
  • Report any acts, incidents, or conditions reflecting the possibility of inappropriate youth-to-youth or staff-to-youth relationships
  • Report any use of physical force and all unusual incidents according to established guidelines
  • Perform other duties as assigned or needed
Required Qualifications
  • Bachelor’s degree from an accredited college or university in counseling, social work, psychology, rehabilitation, special education, or a related human services field
  • Advanced verbal and written communication skills
  • Strong organizational, analytical, documentation, and time-management skills
  • Understanding of group leadership, group dynamics, and experiential education
  • Ability to work independently while collaborating effectively with youth, families, clinical professionals, and outside agencies
  • Ability to pass required background checks and pre-employment screenings
Preferred Experience
  • Two or more years of experience working with children or adolescents with serious emotional, behavioral, or substance use needs
  • Candidates with a master’s degree in an appropriate field may be considered without two years of professional experience
  • Experience working in a Medicaid provider environment
  • Experience with the Department of Children’s Services, juvenile justice agencies, residential programs, assessment centers, or other child welfare settings
  • Experience with case-record management and group-work techniques
Other Requirements
  • Physical Demands: Must be in good physical health and emotionally stable. Must be able to move throughout the property, participate in youth activities, and transport youth or staff when required. Must be able to meet applicable health requirements and successfully complete the PAR physical techniques evaluation and written certification examination.
  • Other: Must maintain an appropriate and valid state driver’s license. Must meet all applicable state criminal background check and pre-employment screening requirements.
