Job summary

Wellnest Emotional Health & Wellness is seeking a Case Manager to join a multidisciplinary team providing community resources, behavioral interventions, and advocacy for children, youth, and their families. Qualifications

  • One to two years’ experience providing case management services to low-income individuals in a community setting.
  • Bachelor's degree from an accredited college or university in social work, human services or a related field.
  • Valid California driver's license and clean driving record per Wellnest requirements.

Responsibilities

  • Collaborate with a multi-disciplinary team to assess client needs and arrange access to health insurance, housing, and child care services.
  • Develop, implement, and monitor service plans addressing personal, social, emotional, mental health, education, and physical needs.
  • Provide guidance to families and care providers on managing behaviors and reducing stressors.
  • Work in community, home, school, and field settings; participate in team meetings and community events.
  • Maintain documentation and complete progress notes in a timely manner.

SCOPE OF RESPONSIBILITY: The Case Manager works as a part of a Multi-Disciplinary team providing community resources, behavioral interventions, and advocacy for children, youth and their family/caregivers who support them. As an integral member of the team, the Case Manager also shares responsibility for the wellness of the client by being on call for crisis with the Intensive Services Division.The Case Manager assists in the development, implementation, and monitoring of a service/treatment plan for each client that identifies the client’s personal, social, emotional, mental health, education, physical and/or linkage needs and goals. Participates with treatment team members in the provision of services to clients, families and/or caregivers. The Case Manager is an active participant in the teaming process and in the Child and Family Team meetings. Case Managers must display tact and be skillful when interacting with external entities such and the Department of Children and Family Services (DCFS), Department of Probation, school personnel, etc. Case Managers will represent the organization at community events.ESSENTIAL FUNCTIONS:Works together with therapist to conduct comprehensive needs assessments with children, youth, adults, and families to assist them in obtaining needed resources such as health insurance, housing, and low-cost child care through referral and linkage.Provides individual and group rehabilitation services under the Department of Mental Health contract or other agency funding.Meets agency’s productivity standards for client service hours.Completes and submits progress notes in a 24-hour timeframe utilizing the agency’s electronic health record system (EXYM) and collaborative documentation while serving clients; discusses and receives proof of insurance / Medi-Cal information in the field and submits to the QMET and/or IS Support Team.Provides guidance and assistance to parents/caretakers on managing child’s behaviors successfully.Provides guidance and assistance to parents/caretakers on managing familial stressors effectively and successfully.Assists family with identifying internal resources to assist with multiple stressors such as extended family members, grandparents.Develops and provide client rehabilitation services which assist in improving, maintaining, or restoring daily functioning at home, school, and community in accordance with treatment goals and in collaboration with treatment team (i.e., therapist, psychiatrist).Provides interagency and intra-agency consultation, communication, coordination and referrals.Identifies and communicates to therapist obstacles preventing families from connecting with program and/or referral services.Attends community meetings as requested.Represents the agency at community resource fairs.Follows up with families that drop out of treatment.Provides services in the communities, in homes, schools and other field-based settings.Participate in trainings and meetings as assigned.Other duties as assigned.REQUIRED QUALIFICATIONS: Experience: One to two years’ experience providing case management services to low-income individuals in a community setting with at-risk populations.Education: Bachelor's degree from an accredited college or university in social work, human services or a related field.CONDITIONS OF EMPLOYMENT: Employee may be asked to pursue additional training when it is determined to be in the best interest of the agency. Employee must have the ability to use a personal vehicle with the required insurance coverage to complete the essential duties of their job and conduct agency business. Employee must maintain a valid California driver’s license and clean driving record that meets Wellnest’s insurance carrier’s criteria for liability coverage.WORKING CONDITIONS: Employee regularly works out in the field and in an office environment.
