Job summary

Laguna Shores Mental Health in Laguna Niguel, CA is seeking a dedicated Case Manager to conduct assessments, develop treatment plans, and coordinate recovery services within a multidisciplinary team.

You will motivate clients to engage in therapy, facilitate plans, and present educational programs to support recovery and personal growth.

This in-person role requires CADC I/CCAPP (optional) and CPR/First Aid, with at least two years of mental health experience.

Qualifications

• Two years’ experience in the mental health field.
• DOJ Fingerprint Live Scan Clearance.
• CPR and First Aid Certification.
• Current CADC I/CCAPP certification (optional).

Responsibilities

• Orient new clients to Laguna Shores Mental Health services and treatment plan.
• Aid clients in setting and achieving short/long-term goals with adaptive skills.
• Liaison with family, friends, and outside resources for continuity of care.
• Educate clients on mental and behavioral health and daily skills training.
• Collaborate with clinical team to address daily concerns and crises when needed.
• Assist clients in developing an aftercare plan for post-program confidence.
• Uphold client confidentiality per policies and laws.
• Perform other duties as assigned.

Job Description

This position is responsible for conducting assessments, treatment planning, counseling, multidisciplinary treatment team consultation and coordination of recovery program services in a mental health setting. The case manager motivates clients to actively participate in their therapeutic programs, and facilitates the implementation, presentation, evaluation and revision of treatment plans. They are also responsible for facilitating/presenting educational programs to clients.

Responsibilities

• Orient new clients to the expectations of, services offered by, and treatment plan she/he will work towards while at Laguna Shores Mental Health.
• Aid clients in setting and achieving short and long-term goals by promoting adaptive coping skills that will set the client up for success.
• Liaison with family, friends, partners, and outside resources to create a supportive environment and ensure continuity of care.
• Educate the clients on mental and behavioral health, promoting healthy resources and daily skills training to provide highest quality person-centered care.
• Work closely with other members of the clinical team in addressing daily concerns, intervening if crisis intervention is required, and offering additional directives for care to provide highest quality person-centered care.
• Assist clients in developing an aftercare plan so that s/he will feel confident upon exiting the program.
• Uphold client confidentiality in accordance with company policies and procedures and local, state, and federal laws and regulations.
• Other duties as assigned.

Minimum Requirements

• Current certification through California Consortium of Addiction Programs CADC I/CCAPP. (Optional)
• Two years’ experience in the mental health field
• Current CPR and First Aid Certification
• DOJ Fingerprint Live Scan Clearance
