Job summary

Trinity Services Inc. in Des Plaines, IL seeks a Case Manager to develop and implement person-centered plans for individuals receiving supports. This role involves leadership in the Community Support Team and ensuring program data drives growth and development for those served.

You will coordinate health, financial matters, and advocacy efforts, while maintaining regulatory compliance and promoting empowerment, community integration, and normalization.

Qualifications

  • Bachelor’s degree in a Human Services field as defined by the Illinois Department of Human Services
  • At least one year of experience supporting individuals with intellectual/developmental disabilities
  • Must meet state and federal requirements for QIDP status
  • Experience guiding diverse support staff and individuals with varying abilities

Position Overview

The Case Manager, formerly titled Qualified Intellectual Disabilities Professional (QIDP), works under the supervision of the Network Director or their designee. This position involves key responsibilities in developing and implementing person-centered plans and strategies for individuals receiving supports and services. The Case Manager is responsible for maintaining and evaluating program data to ensure the growth and development of individuals supported. The role also includes leadership responsibilities, including communicating agency policies and promoting service values such as community integration, empowerment, and normalization.

Essential Functions
Person-Centered Planning & Individual Support
  • Guide and lead the Community Support Team in delivering individualized supports and services aligned with each person’s desired goals and outcomes.
  • Conduct Personal Outcome Measure interviews annually or as needed/requested.
  • Ensure individuals' preferences and goals are accurately reflected in person-centered documents and updated as required.
  • Facilitate Community Support Team meetings to celebrate accomplishments, develop and update Flourish Plans, and address care-related decisions.
  • Provide direct support to people in the home and community to help them achieve their goals and outcomes.
  • Promote natural support networks to assist with the development and maintenance of personal relationships.
Documentation & Compliance
  • Complete all required service documentation in compliance with DHS and Trinity timelines.
  • Monitor and document monthly progress toward identified outcomes, making service adjustments as needed under Network Director guidance.
  • Complete Service and Support Agreements with people supported and their teams.
  • Ensure health and well-being are coordinated in line with DHS regulations, agency policy, physician recommendations, and people supported preferences.
  • Understand and apply regulatory standards including, but not limited to, IDHS rules and requirements.
  • Maintain QIDP status through 12 hours of annual continuing education, including six hours from non-Trinity sources.
  • Complete initial and ongoing medication authorization training for residential QIDPs.
Health & Financial Coordination
  • Assist in coordinating health-related needs, including hospital or nursing home care and follow-ups.
  • Help manage benefits and financial matters such as Social Security, Medicaid/Medicare, and bank accounts.
  • Identify and coordinate additional supports that may enhance a person’s health and well-being, including referrals to support services or consultants.
Team Leadership & Staff Development
  • Provide coaching and mentoring to Independent Living Coaches and other staff as needed.
  • Offer training related to person-centered planning, outcome tracking, and implementation of behavior support plans.
  • Make recommendations to the Network Director for hiring, terminating, promoting, and evaluating Independent Living Coaches.
  • Provide temporary supervision of residential sites and participate in on-call responsibilities when necessary.
Advocacy & Team Collaboration
  • Advocate for the rights and services of people supported, including participation in Human Rights Committee meetings.
  • Involve individuals in advocacy and decision-making processes to the extent they are able and desire.
  • Recommend additional services or supports beneficial to the wellbeing of each person supported.
  • Coordinate and facilitate access to external supports and services (e.g., consultants, funding, referrals).
Organizational Participation & Improvement
  • Attend and contribute to programmatic and organizational meetings and trainings.
  • Make suggestions for program or organizational improvement and share ideas with colleagues.
  • Seek ongoing opportunities for personal development and continued learning.
  • Perform other duties as assigned based on the needs of the organization.

Physical Demands
  • Lift over 40 lbs from floor to waist
  • Push/pull up to 75 lbs and wheelchairs up to 200 lbs
  • Complete deep knee squats and other physical movements including standing, stooping, crouching, kneeling, climbing, crawling, and repetitive motion
  • Operate motor vehicles and perform tasks while standing or sitting for extended periods
  • Use hands and fingers for computer and phone operation
