SUMMARY

The Case Manger serves as a liaison between the residents, assigned facility staff, the resident’s family, county placement workers, and any other outside agency workers by being the primary point of contact for the Agency. This position is responsible for all aspects of coordination of client clinical service related to clinical, medical, court, and family interaction and the documentation of those functions.

DUTIES AND RESPONSIBILITIES

• Working collaboratively with the Facility Social Worker and Administrators to ensure that the needs of the clients are being met within the assigned caseload.
• Acting as a liaison between the residents, assigned facility staff, the resident’s family, county placement workers, and any other outside agency workers.
• Scheduling, confirming, and arranging of weekly visitations.
• Maintaining the clinical and archival files for each assigned resident.
• Working with the Intake Coordinator and Administrator to ensure all the necessary paperwork is obtained and completed on new residents.
• Monitor medical insurance and billing issues regarding the assigned residents.
• Maintaining a petty cash budget for client medications, client transportation, etc.
• Coordination and attendance of all meetings regarding assigned residents (i.e., Case conferences, IEPs, Staff meetings, Stabilization meetings, Wrap meetings, TBS meetings, etc.).
• Providing transportation to and attending court dates of all assigned residents.
• Overseeing the maintenance, storage, and tracking of each assigned residents’ medication.
• Tracking medical appointments of all assigned residents.
• Completion of the assigned sections of the Quarterly reports.
• Coordination and communication liaison for crisis situations involving assigned residents, their families, County placement workers, Wrap workers, and the psychiatric hospital.
• Submit required documentation in timely manner, in compliance with state, county and company policies and maintain quarterly productivity of 35%.

OTHER DUTIES AND RESPONSIBILITIES

• Completing and submitting the Monthly Bed Funding sheets to the Chief Financial Officer.
• Performs all other tasks as instructed by the supervisor, consistent with the above duties.

Qualifications

Must have

• Related BA/BS or 4 years’ experience, or
• Non-related BA/BS, PLUS 2 year experience, or
• 60 college units (minimum 15 units in behavioral science), PLUS 2 years’ experience.

Must be able to interrupt physical altercations, respond to life-threatening crises, hear interactions among residents or call for assistance from staff, to walk, run, climb, lift, speak and use their mental faculties to make appropriate judgments in all areas that affect major life activity of child care protection.

A valid California Driver's License is required.

WORKING CONDITIONS

Working conditions are normal for an office environment. Work may require frequent weekend and evening work. Work may require overnight travel.

NON-EXEMPT STATUS

The position is a non-exempt as defined under Order 5-2001, Department of Industrial Relations, Public Housekeeping Industry.

DOCUMENTATION OF HOURS

Time cards are to be completed and submitted for actual hours worked. This is a non-exempt position and documentation of actual hours worked is required to track hours for sick days and vacation days.
