Spaulding Nursing and Therapy Center Brighton, Inc. is hiring for a full‑time day shift position focused on case management and discharge planning.
The role involves intake interviews, evaluating medical and psychosocial needs, and coordinating services with a multidisciplinary team. The ideal candidate has MA licensure (RN/PT/OT/SLP/RT or LCSW/LICSW) and 2–3 years of case management or discharge planning experience.

This is a full‑time, Monday to Friday day shift position. The organization offers a generous sign‑on bonus of up to $10,000. The bonus details will be discussed during the interview process.
Responsible for completing intake interviews; determining the need for medical and psycho‑social evaluations; reviewing therapist and physician evaluations, care objectives, and plans; coordinating services being provided; advocating for needed services and entitlements.

Education: Bachelor’s Degree in a related field of study required. Experience in lieu of a degree is not accepted.
Licenses and Credentials: RN, PT, OT, SLP, RT or LCSW/LICSW required.
Experience: 2–3 years in case management, utilization review, or discharge planning preferred.
