Encompass Health Corporation is seeking a Case Manager to coordinate patient care from admission to post-discharge. You will work with an interdisciplinary team to develop and adjust treatment plans, ensure timely discharges, and support patient education throughout the rehabilitation journey.

The role emphasizes regulatory knowledge, quality metrics, and collaboration with clinicians, payors, and families to achieve meaningful outcomes in a community setting.

Qualifications

• Must have an RN license with an associate degree minimum.
• CCM or ACM certification required or must be obtained within two years of entry.
• Bachelor's degree and graduate degree preferred.

Responsibilities

• Coordinate interdisciplinary care plans from admission to discharge.
• Establish tentative discharge plans and contingency plans with the team.
• Educate patients and families on rehabilitation and case manager role; communicate plan across providers.
• Attend Acute Care Transfer (ACT) meetings to identify trends and improve outcomes.
• Review case management data and implement actions to improve quality and timeliness.

• Work withinterdisciplinaryteam,guidingtreatmentplansbasedonpatientneedsand preferences.

• Performassessmentofgoalsandcompletecasemanagementaddendumwithin48hoursof admission.
• Educatepatient/familyonrehabilitationandCaseManagerrole;establishcommunication plan.
• Scheduleandfacilitatefamilyconferencesas needed.
• Assistpatientwithtimelyprocuring/planningofresourcestoavoiddischargedelaysor issues.
• Monitorcompliancewithregulationsfororthoticsandprostheticsorderingand payment.
• Makeappropriate/timelyreferrals,includingdocumentationtopostdischarge providers/physician
• Ensureaccuracyofdischargeandpayor-relatedinformationinthepatient record
• Participateinutilizationreviewprocess:datacollection,trendreview,andresolution actions.
• Participateincasemanagementon-callscheduleas needed.

Qualifications

License or Certification:

• Must be qualified to independently complete an assessment within the scope of practiceofhis/herdiscipline(forexample,RN,SW,OT,PT,ST,andRehabilitation Counseling).
• Iflicensureisrequiredforone'sdisciplinewithinthestate,individualmustholdan active license.
• MustmeeteligibilityrequirementsforCCM®orACM™certificationuponentryintothis position OR within two years of entry into the position.
• CCM®orACM™certificationrequiredORmustbeobtainedwithintwoyearsofbeing placed in the Case Manager II position.

Minimum Qualifications:

• For Nursing, must possess minimum of an Associate Degree in Nursing, RN licensure with BSN preferred. A diploma is acceptable only in those states whose minimum requirementforlicensureorcertificationisadiplomaratherthan an associate degree.
• Forallothereligiblelicensedorcertifiedhealthcareprofessionals,mustpossessa minimum of a bachelor's degree and graduate degree is preferred.
• 2yearsofrehabilitationexperience preferred.
