Job summary

The Salvation Army in Raleigh, NC is seeking a full-time Case Management Specialist for the Family Shelter program. You will interview and provide structured case management services to an assigned client caseload, focusing on Social Services Benefits and guiding clients through the application process.

You will assess each client’s history to tailor program plans, develop goals, and evaluate progress through weekly meetings while ensuring compliance with funding requirements.

Qualifications

• Bachelor's degree in Social Work, Behavioral Science, or related field; three years progressively responsible experience.
• Or any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.
• Certifications: Valid State Driver’s License

Responsibilities

• Determines social services benefits that clients are eligible for and guides them through the application process.
• Understands client history to determine the most effective program plan and develops comprehensive goals and evaluates progress via weekly meetings.
• Ensures compliance with funding requirements.
• Enrols families in social services benefits and assists with applying for benefits; SOAR certification to assist with the process.
• Outsources clients to additional counseling resources and collaborates with external case management professionals to meet holistic needs.
• Meets regularly with clients to discuss progress and documents records; acts as liaison to other agencies to coordinate assistance.

ABOUT THIS OPPORTUNITY

Schedule/Hours: Full-time, Monday – Friday, including some weeknights

This position is responsible for:

Interviews, accepts, and provides structured case management services for an assigned caseload of clients participating in the Family Shelter program, with a focus on Social Services Benefits; determines social services benefits that clients are eligible for and guide them through the application process; understands the uniqueness of client's history in order to determine the most effective program plan; develops comprehensive program plan/goals and evaluates client's progress by conducting weekly meeting sessions; ensures compliance with funding requirements.

Key Responsibilities:

• Ensures families participating in program are enrolled in social services benefits. Understands the eligibility requirements and enrollment process.
• Assists families with applying for appropriate social service benefits; becomes SOAR certified to assist with the process.
• Outsources clients to additional counseling resources if needed; maintains awareness of progress with external case management professionals, working as a team to meet the holistic needs of the individual.
• Meets regularly with clients to discuss and evaluate their progress, feelings, impressions, changes and personal growth/development; prepares accurate and up-to-date records documenting the same.
• Serves as liaison to other agencies to obtain/coordinate assistance on client's behalf; builds and maintains professional working relationships with community agencies.

Physical Requirements and Working Conditions:

• Ability to meet attendance requirements, which includes working weeknight evenings.
• Ability to read, write, and communicate the English language effectively.
• Ability to communicate clearly and effectively both orally and in writing.
• Ability to think clearly and quickly to maintain control of client caseload.
• Ability to effectively and efficiently work on multiple cases at the same time without becoming frustrated or disorganized.
• Limited amount of physical effort required associated with walking, standing, lifting, and carrying light objects (less than 25 lbs.) 5-10% of work time.
• Duties are usually performed seated. Sitting may be relieved by brief or occasional periods of standing or walking.
• Work is performed in a day shelter environment where there may be exposure to physical health risks and unsanitary conditions. Work involves driving a vehicle to off-site meetings and home visits where there may be physical discomforts associated with changes in weather or discomforts associated with noise, dust, dirt, and the like.
