$21.00-$25.00 per hour (based on experience)
The Targeted Case Manager plays a vital role in helping guests transition from homelessness to stable, permanent housing. Working within our low-barrier shelter, you'll partner with individuals experiencing complex challenges—including mental illness, substance use disorders, trauma, and chronic homelessness to develop individualized housing plans and connect them with the resources they need to succeed.
This position provides supportive housing services and will deliver Section 13 and/or Section 17 MaineCare Targeted Case Management services for eligible participants.
You’ll work alongside an experienced multidisciplinary team committed to reducing barriers, promoting recovery, and helping people achieve long-term housing stability.
