Job summary

The Fire Department, City of New York (FDNY) seeks a FULL TIME Case Management Nurse in the Bureau of Health Services to support the WTC Health Program for active and retired FDNY members. The role emphasizes comprehensive case management, documentation of encounters, and collaboration with physicians to monitor treatment plans.

Reporting to the WTC Nurse Supervisor, you will assess and reassess client function, ensure medical needs are addressed, and advocate for patients when necessary

Qualifications

• Valid New York State RN license with BSN and 3 years of clinical nursing experience.
• Valid New York State RN license with ASN and 5 years of clinical nursing experience.
• Education and/or experience equivalent to above; RN license required.

Responsibilities

• Maintain a comprehensive case management system for the FDNY WTC Health Program.
• Document all client encounters and nursing summaries.
• Evaluate and document progress of therapeutic programs.
• Consult with physicians to coordinate treatment plans and advocate for clients.
• Coordinate initial assessments and ongoing reassessments of functional status.
