Job summary

Oak Valley Hospital District is seeking a dynamic Case Manager for our Nursing Administration department to plan, organize, direct, coordinate and evaluate identified managed care patients according to pre-established criteria to maintain high quality patient care.

The Case Manager will ensure the department meets all regulatory requirements related to DHS, The Joint Commission, and others as appropriate, working collaboratively with other managers to meet goals and solve problems.

Qualifications

• Graduate from accredited school of Nursing.
• Minimum of three (3) years’ clinical experience in a healthcare setting.
• Utilization review experience is preferred.
• Maintains current California Registered Nurse Licensure or has graduated from an accredited RN program and works directly under the supervision of a California licensed RN.
• Prefer California Public Health Certification.

Responsibilities

• Plan, organize, direct, coordinate and evaluate identified managed care patients following pre-established criteria to maintain high quality patient care.
• Work collaboratively with other managers to meet goals and solve problems and ensure regulatory compliance for DHS, The Joint Commission, and others as appropriate.

Bachelor’s in science, Nursing (BSN) preferred
