Qualifications

• Three years in case management and two years in a leadership role.
• RN or BSN required; case management certification preferred.
• Must demonstrate critical thinking, effective communication, decisive judgment, and the ability to work independently in high‑pressure environments.
• Knowledge of hospital information systems and performance improvement techniques.

Responsibilities

• Oversee and coordinate daily case management and discharge planning to ensure quality patient care, regulatory compliance, and efficient patient flow.
• Collaborate with multidisciplinary teams, interpret payer guidelines, and apply clinical decision‑making.
• Ensure discharge plans meet state and federal regulations using InterQual or Milliman guidelines.
• Monitor, improve processes, performance, and staff development.
