Qualifications

• Minimum of 3 years of hospital-based Case Management experience required
• Experience with Utilization Review and Discharge Planning required

• Lead the Case Management team within the rehabilitation hospital setting
• Collaborate with leadership to align case management with patient-centered care and clinical excellence
