Responsibilities
  • The Case Management Assistant II will support the Care Coordination department at California Pacific Medical Center's Davies Campus. This is a non-clinical, minimally patient-facing role focused primarily on skilled nursing facility admissions and intake rather than direct patient care.
  • Responsibilities include coordinating transition-of-care and discharge activities, gathering and organizing required information, communicating with care providers, skilled nursing facilities, families, insurance contacts, and external vendors, and helping ensure timely placement and discharge.
  • The assistant will manage multiple priorities, maintain accurate documentation, follow established workflows, elevate questions when needed, and work independently in a fast-paced environment.
  • Strong communication, organization, time management, computer, and word processing skills are required.
Required Skills
  • High school diploma, GED, or equivalent education
  • 1+ years of recent relevant experience
  • Experience with healthcare operations, care coordination, or related administrative workflows
  • Ability to coordinate transition-of-care and discharge-related activities across multiple stakeholders
  • Proficiency in intermediate PC, word processing, and electronic documentation
  • Strong oral and written communication skills
  • Ability to work independently with minimal direction
  • Ability to prioritize competing tasks in a fast-paced environment
  • Ability to exercise discretion and seek input when appropriate
