Job summary

HCA Florida Largo Hospital is seeking a Case Management Assistant to support RN case managers and social workers with discharge planning and post-acute placement coordination.

The CMA will identify discharge barriers, provide administrative support, and help ensure smooth transitions for patients, contributing to high-quality patient care and an efficient case management workflow.

Qualifications

• Associates degree preferred.
• Minimum of one year experience in a health care setting.
• Medical terminology knowledge required.
• Healthcare professional (PCT, CNA, EMT) preferred.
• Ability to communicate effectively in oral, written and electronic formats.
• Demonstrates ability to establish collaborative working relationships.

Job Summary And Qualifications

The case management assistant (CMA) will work under the direction of the RN case manager or social worker to assist with facilitating patient discharges requiring post-acute placement or services and will identify and report barriers to discharges. The CMA will provide administrative support in Case Management functions.
