• Provide personalized care and support to clients, including assistance with daily living activities, medication management, and mobility and transfer assistance.
• Establish and maintain positive relationships with clients, families, and healthcare professionals.
• Conduct initial and ongoing assessments of clients' needs and preferences to develop individualized care plans.
• Administer medications, treatments, and other therapies as instructed.
• Maintain accurate and detailed records of client care and services.
• Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination.
• Participate in ongoing training and education to stay current with industry best practices and industry regulations.
