JOB REQUIREMENTS: To perform this job successfully, an individual must

be able to perform each essential duty satisfactorily. The Caregiver

will be trained in the provision of care services and in each skill that

is assigned which can include but not limited to: Assistance with

bathing, dressing, grooming, eating, mobility, toileting, and

transferring Changing of consumer(s) bed, laundering of bed linens and

consumer(s) personal clothing Medically orientated tasks (medication

assistance, tube feedings, skin care, mechanical lifts, etc) as assigned

Care of eyeglasses and hearing aids Light cleaning in essential areas of

home during personal care activities Meal preparation and meal serving

Providing companionship and activities (including transportation as

assigned) Promptly reporting changes in consumer's condition to the

designated supervisor Completion and submission of Records of Care in a

thorough, accurate, and timely manner Maintain confidentiality of

consumer's information Adhere to and abide by all policies and
