Job summary

Fountaingrove Lodge, part of Oakmont Management Group, in Santa Rosa, CA, seeks a Care Provider to assist residents with daily living and supportive care, promoting independence and dignity for seniors. Join a team dedicated to authentic, compassionate service in a nurturing community.

Responsibilities include ADL assistance, prompt response to resident needs, communicating changes to supervisors, maintaining safety, and collaborating across departments. Training is provided where needed.

Qualifications

• High School Diploma or GED required or equivalent.
• Must pass Criminal Background check and Health Screening tests.
• Ability to communicate effectively with residents, families, and staff.
• 18 years of age or older and able to meet physical requirements.

Responsibilities

• Assist residents in Activities of Daily Living (ADL) per care plan.
• Respond promptly to residents' needs while preserving dignity and safety.
• Report changes in resident condition to a supervisor immediately.
• Maintain a safe, secure environment following safety standards.
• Promote teamwork through cooperative interactions with staff.

The Care Provider is responsible for providing personal assistance and daily care and services related to the residents in order to enhance the residents’ independence and quality of life, in accordance with the company’s mission and values.

Responsibilities:

• Assist residents in their Activities of Daily Living (ADL) as specified in the resident’s service and care plan. This includes eating, bathing, dressing, toileting, transferring (walking), and continence.
• Respond to residents' needs promptly while maintaining residents' self-respect, dignity, safety, and confidentiality.
• Communicate any observed or suspected resident change of condition to a supervisor immediately.
• Maintain a safe and secure environment for all staff, residents, and guests, following established safety standards.
• Encourage teamwork through cooperative interactions with co-workers and other departments

Qualifications

• Experience preferred but not necessary; we will train the right candidate
• Must be at least 18 years of age
• High School Diploma or GED
• Strong desire to work with the elderly and care for their needs
• Possess written and verbal skills to interact and communicate effectively with employees, supervisors, physicians, healthcare professionals, residents, and their families.
• Must pass a Criminal Background check and Health Screening tests, including physical and TB Tests
• For the health and safety of our team members and residents, Oakmont Management Group may require team members to vaccinate, participate in daily screening, surveillance testing, and to wear face coverings and other personal protective equipment (PPE) to prevent the spread of the COVID-19 or other communicable diseases, per regulatory guidelines.
