Job summary

At Home Quality Care is seeking an in-home caregiver to support clients in their homes under supervision of the Home Services Coordinator. You will assist with daily living activities, provide companionship, and help with housekeeping and medication reminders.

The role requires reliable, compassionate individuals who can read and follow a written plan of care, communicate effectively with clients and the care team, and perform tasks with minimal supervision.

Qualifications

Responsibilities

• Observe client functioning and report changes to supervisor.
• Perform tasks within assigned scope.
• Attend to client requests promptly.

• Provide companionship, socialization, and housekeeping services.
• Assist with medication reminders including prompts and safe handling.
• Assist with meal planning, shopping, and meal preparation.
• Maintain a clean and safe environment; clean bathrooms and kitchen.
• Run personal errands, laundry, vacuuming, dusting, and change bed linens.
• Follow infection control policies and hand washing.
• Maintain communication with supervisory staff and interdisciplinary team.
• Document care and observations accurately.
• Treat clients and families with respect and maintain confidentiality.
• Accept assignments and attend in-service programs.

Job description

The Caregiver works under the supervision of the Home Services Coordinator and performs tasks that contribute to the client's comfort and safety in the home.

Qualifications

Essential Functions/Areas Of Accountability

• Observation of client functioning and reporting changes to his/her supervisor or employer.
• Functions within the limitations of the individual assignment.
• Attends to the client's requests promptly.
• Assistance with activities of daily living and personal care.
• Provides companionship, socialization, and housekeeping services.
• Assists with medication reminding. Which can include:
• Inquiries as to whether medications were taken.
• Verbal prompting to take medications.
• Handing the appropriately marked medication reminder container to the client.
• Opening the appropriately marked medication reminder container for the client if the client is physically unable to open the container.
• Assists with meal planning, purchase of food, and food preparation.
• Assists clients to live in a clean and safe environment:
• Cleans bathrooms and kitchen, including stove and refrigerator.
• Personal errands, per client and Agency request.
• Laundry.
• Vacuuming and dusting.
• Changing bed linens.
• Knows and follows infection control policies including proper hand washing techniques.
• Maintains appropriate communication with supervisory staff and with all members of the interdisciplinary team.
• Documents care given and pertinent observations accurately as directed by the home service coordinator.
• Treats clients and families with respect. Maintains confidentiality.
• Accepts and fulfills assignments with the Agency. Exercises judgment in accepting assignments.
• Attends required in-service programs per Agency policy.

CAREGIVERS SHALL NOT ACT IN THE FOLLOWING CAPACITIES

• Become or act as a Power of Attorney
• Be involved in any financial transactions of the client outside of contracted services. In such cases, the home services worker shall follow agency policies in regard to securing receipts for items purchased and ensuring both client and worker signatures documenting those expenditures.
• Perform or provide medication setup for a clientWhen a client has skilled skin care needs or skilled dressing that will need attention before, during, or after bathing, the client shall be in the care of a licensed home health agency or nursing home to meet those specific needs.
• Provide skilled personal care services defined as the following: Skilled Personal Care – personal care that may be provided only by a home health aide, as defined in this Section, or an individual who is a certified or licensed health care professional under the laws of the State of Illinois.

Required experience

• 1 year of caregiver experience/ CNA preferred: 1 year
