Job summary

360 Wellness Village seeks a Caregiver Case Manager to support seniors and people with disabilities, coordinating wraparound home-based services to keep clients safe at home. You will report health, housing, or safety changes to management and help with daily tasks to improve living conditions.

Responsibilities include hygiene assistance, mobility support, meal prep, light housekeeping, medication dispensing, and companionship to enhance clients' well-being and independence.

Qualifications

• Experience as a Caregiver/Home Health Aide or similar role preferred.
• First aid and CPR certification.
• Knowledge of basic housekeeping tasks and cooking skills.
• Ability to adhere to health and safety guidelines.
• Excellent communication and interpersonal skills.
• Able to lift heavy objects; compassionate, respectful and ethical.

Responsibilities

• Assist with personal hygiene needs and dressing.
• Assist with mobility, walking, and physical therapy exercises.
• Prepare meals and snacks.
• Light housekeeping activities.
• Dispense medication.
• Provide companionship and assist with errands/shopping.
