Charis Hearts, Inc. is a statewide provider of homecare services through the TennCare CHOICES program.

We offer flexible scheduling, weekly direct deposit, health benefits, and fast onboarding to support caregivers delivering non-medical in-home care. Caregivers provide client-centered support in clients' homes, perform wellness checks, assist with admissions, and may train others on ADLs, all while maintaining strict confidentiality and adhering to service plans.

Qualifications

• Must have strong communication skills—oral and written.
• Reliable transportation to client locations.
• Must be at least 18 years old.
• Valid State ID/Driver's License.
• Willingness to attend training sessions.

Responsibilities

• Provide non-medical in-home and community care per the Client Service Plan.
• Perform client wellness checks to ensure well-being.
• Assist clients with admission documents as needed.
• Train other caregivers on Activities of Daily Living (ADLs).
• Maintain confidentiality of client records and information.

Caregiver Responsibilities

Caregivers at Charis Hearts provide non-medical in-home and/or community care based on the Client Service Plan. Responsibilities may include:

• Client Support & Care
  Perform client wellness checks to ensure their well-being.
  • Assist clients with completing admission documents as needed.
  • Help train other caregivers on Activities of Daily Living (ADLs) in the client's home.
  • Provide non-medical support to agency service recipients.
• Compliance & Documentation
  Be familiar with and follow all rules, policies, and procedures related to service delivery.
  • Submit all required documentation accurately and on time.
  • Adhere to the individual service plan for each recipient.
  • Maintain strict confidentiality of all client records and information unless authorized by written consent.
• Safety & Reporting
  Immediately report any incidents of injury involving the service recipient or yourself to your supervisor.
  • Report any suspected abuse or neglect to the appropriate authorities and your supervisor.

To be considered for this role, applicants must meet the following qualifications:

Client Prefers a Male Caregiver

• Must be at least 18 years old.
• Must have valid State ID/ Drivers License.
• Strong communication skills – both oral and written.
• Reliable transportation to and from client locations.
• Compassionate and caring nature with a genuine desire to help others.
• Honesty and integrity in all interactions.
• Willingness to attend training sessions provided by referring agencies and/or the Tennessee Department of Mental Health & Developmental Disabilities (MHDD).
