Job summary

Live Well Home Care in Wisconsin is seeking a compassionate caregiver to support clients with daily living activities and personal care in their home. You will assist with ambulation, grooming, meals, light housekeeping, and medication reminders as outlined in each care plan.

You will provide companionship, ensure safety, and help with transportation to appointments or errands. Collaboration with the care team and supervision by the Branch Manager ensures high-quality, compliant care.

Responsibilities

• Assist with activities of daily living including ambulation, bathing, dressing, feeding, toileting, transferring
• Provide personal care and ensure client safety and security in the home
• Prepare and assist with meals, housekeeping, laundry, and routine chores as per care plan
• Provide companionship, social interaction, and emotional support to clients
• Offer respite care to families per the care plan and under supervision
• Run errands and assist with shopping as needed per care plan
• Aid in following written diet plans and reinforce diet maintenance as identified
• Escort clients to medical appointments, appointments, and outings as specified in the care plan
• Assist with communication by writing or typing correspondence for clients
• Collaborate with Client Care Team and Branch Manager to share input and suggestions
• Follow policies, procedures, and care plans; monitor supplies and resources
• Report unsafe conditions to Branch Manager and observe clients and environments

Duties/Responsibilities:

Assist with the activities of daily living and personal care including:

• ambulation
• bathing
• dressing
• exercise
• feeding
• hair care
• medication reminding
• mouth care
• nail care
• positioning
• shaving
• skin care
• toileting
• transferring
• Ensure client’s safety and security by supervising the home environment.
• Teach/perform meal planning and preparation, routine housekeeping activities such as making/changing beds, dusting, vacuuming, washing floors, cleaning kitchen and bathroom, and laundry.
• Provide companionship including social interactions, conversations, emotional reassurance, and encouragement of activities that stimulate the mind.
• Provides respite care for families in accordance with care plans.
• Perform/assist with essential shopping/errands, which may include handling the client’s money in accordance with the care plan and under the observation of the Branch Manager.
• Assist clients with following a written, special diet plan and reinforcement of diet maintenance as identified on the care plan.
• Escort clients to medical facilities, errands, shopping, and outings as specified in the care plan.
• Assist clients with communication by writing or typing correspondence for them.
• Participate with Client Care Team (Branch Manager and Regional Leadership) by providing input and making suggestions.
• Ensure service is delivered in accordance with all relevant policies, procedures, and practices.
• Monitor supplies and resources.
• Follow the written care plan.
• Carry out duties as assigned by the Branch Manager.
• Observe clients and their environments and reports unsafe conditions to Branch Manager.
