Charis Hearts, Inc. is a statewide homecare provider serving TennCare CHOICES clients. We are seeking compassionate caregivers to support clients in their homes across Tennessee.

This role emphasizes dignity, accuracy in documentation, and adherence to service plans. Responsibilities include client wellness checks, ADL support, and ensuring timely completion of admission documents. Qualifications

• Must be at least 18 years old.
• Must have valid State ID/ Drivers License.
• Strong communication skills – both oral and written.
• Reliable transportation to and from client locations.
• Compassionate and caring nature with a genuine desire to help others.
• Honesty and integrity in all interactions.
• Willingness to attend training sessions provided by referring agencies and/or the Tennessee Department of MHDD.

Responsibilities

• Provide non-medical support to clients in their homes.
• Perform client wellness checks and monitor well-being.
• Assist clients with completing admission documents as needed.
• Help train other caregivers on ADLs in the client’s home.
• Adhere to the individual service plan and document required information.
• Maintain client confidentiality and report any incidents promptly.
• Report any suspected abuse to authorities and supervisor.

Caregiver Responsibilities

• Client Support & Care Perform client wellness checks to ensure their well-being.
• Assist clients with completing admission documents as needed.
• Help train other caregivers on Activities of Daily Living (ADLs) in the client's home.
• Provide non-medical support to agency service recipients.
• Compliance & Documentation Be familiar with and follow all rules, policies, and procedures related to service delivery.
• Submit all required documentation accurately and on time.
• Adhere to the individual service plan for each recipient.
• Maintain strict confidentiality of all client records and information unless authorized by written consent.
• Safety & Reporting Immediately report any incidents of injury involving the service recipient or yourself to your supervisor.
• Report any suspected abuse or neglect to the appropriate authorities and your supervisor
