Summary

• Position type: Non‑exempt, full‑time or part‑time, hourly role.
• Scope of role: Provide direct support within a single program or unit, typically supporting 4–6 individuals in residential settings or up to 10 individuals in periodic programs.
• Key responsibilities: Deliver support per individual service/program plans; assist with daily activities (personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living).

Essential Job Functions

• Direct support: Assist with socialization, behavioral development, personal care, housekeeping, recreational activities, transportation, community orientation, shopping, financial management, citizenship, and other activities of daily living.
• Records: Accurately document progress and activity; review records and logs; maintain confidentiality.
• Management of individuals’ assets: Make cost‑effective purchases within budget and according to individual preferences.
• Relationships: Maintain healthy professional relationships with individuals, families, guardians, and case managers; greet family members and visitors; implement Sevita customer service standards.
• Reporting: Report any allegations of abuse or neglect and any incidents in accordance with Sevita policy and external requirements.
• Rights: Maintain confidentiality, respect the rights of persons served, practice universal precautions, and assist individuals in exercising their rights.
• Health care: Appointments, medication administration, general health care, medical supplies and equipment, meal preparation – all to be performed accurately and with safety in mind.
• Employment responsibilities: Attend orientation and ongoing training, volunteer for staff meetings, adhere to scheduling, maintain teamwork, and comply with workplace safety policies.
• Maintenance: Transport individuals, perform housekeeping, make minor repairs, maintain safety, and report any hazards.
• Other: Perform other duties as required and use Mobile Punch Authorized for timekeeping.

Minimum Knowledge and Skills Required for the Job

Education and Experience

• High school diploma or equivalent preferred.
• Six months of experience in human services preferred.
• Must be 18 years of age.
• Working knowledge of computers.

Certificates, Licenses, and Registrations

• Valid driver’s license in good standing.
• Car registration and vehicle insurance if providing transportation for individuals receiving services.
• All state‑required training and certification completed within mandated timeframes.
• Maintain valid driver’s license, meet all insurance requirements, and be approved by management to be a driver for the company.

Work Environment

• Works at the program location and may accompany individuals into the community.

Physical Requirements

• Heavy work: Exert up to 100 pounds of force occasionally, 50 pounds of force frequently, and 20 pounds of force constantly to move objects.

• Employees must cooperate with licensee and Department of Human Services staff during inspections, inquiries, or investigations.
• All employees must obtain drug testing, criminal background checks, and other required screenings before and during employment.
• Employees at least 18 years of age are required to comply with all training and proficiency demonstrations.
