Job summary

WCHN Affiliates is seeking a CNA/Home Health Aide to assist clients at home with ADLs/IADLs according to individualized care plans and CNA scope of practice. You will support personal care, meal prep, mobility, medication prompts, and companionship across client homes in the Nuvance Health Caregivers at Home program.

The role requires at least 1 year of CNA/home health aide experience, a valid driver license, and ability to travel 30 miles or more to cover shifts.

Qualifications

• Experience: Minimum 1 year as CNA, home health aide, patient care tech, or caregiver.
• Preferred: Graduate of nurse aide/home health aide or patient care tech program; or 1 year clinical nursing training.
• Preferred Education: Certified Nursing Assistant HS Graduate or Equivalent

Responsibilities

• Assists with personal care activities (bathing, oral hygiene, feeding, dressing).
• Assists with exercises, ambulation, transfer; uses canes, walkers, crutches, lifts.
• Prepares meals and assist with feeding; follows dietary restrictions and maintains sanitary kitchen conditions.
• Reminds client to self-administer medications and prompts as needed.
• Identifies and reports changes in client condition.
• Provides basic homemaking and laundry assistance in the home.
• Follows agency policies and procedures for care activities.
• Maintains safe, orderly home environment and protects client information; follows infection control policies.
• Provides companionship and engages in activities of interest.
• Understands light pet care (e.g., dog/cat); may assist with transportation to appointments.
• Travel to cover shifts as needed; maintains professional appearance and communication with clients/families.
• Performs compliance duties and other duties as assigned.
• Maintains all duties in accordance with CNAs scope of practice.

Job description

Summary

Provides assistance to clients of Nuvance Caregivers at Home (NHCH) , with activities of daily living (ADLs) and instrumental activities of daily living (IADLs) as directed on clients individual care plans, within the guidelines of the certified nursing assistant (CNA) scope of practice and according to the NHCH policies and procedures.

Responsibilities

• 1. Assists the client with personal care activities including bathing, oral hygiene, feeding and dressing.
• 2. Assists the client with exercises, ambulation, and transfer activities. This includes knowledge in use of devices such as canes, walkers, crutches, and lifts.
• 3. Independently assists the client with meal preparation/cooking and/or feeding. Must have working knowledge of special diets, diet restrictions, and techniques to maintain sanitary kitchen conditions to prevent food-borne illnesses.
• 4. Assists the client with medication prompts to remind the client to self-administer his/her medications.
• 5. Identifies and reports changes in client condition.
• 6. Independently provides basic homemaking and laundry assistance for client in the home.
• 7. Follows policy and procedure for assigned client care activities.
• 8. Maintains a safe and orderly work environment in the clients homes. This includes maintaining professional boundaries and integrity of client financial and personal information in clients homes, as well as following network/agency infection and prevention policies and procedures in clients homes.
• 9. Provides companionship and engages in activities of interest to the client.
• 10. Assists the client with light pet care as needed, such as walking a dog, feeding a dog/cat, changing a cat litter pan.
• 11. Transporting the client to appointments and errands.
• 12. Fulfills all compliance responsibilities related to the position.
• 13. Performs other duties as assigned.

Education Skills Experience

• Experience: Minimum 1 year experience working as a CNA, home health aide, patient care technician or caregiver
• Preferred Experience: Graduate of nurse aide, home health aide or patient care tech program; or minimum of one year of clinical nursing training. Experience working as an aide in home care setting
• Preferred Education: Certified Nursing Assistant HS Graduate or Equivalent

• Valid Driver License. Satisfactory Driving Record (Meets requirements of NHCH policy)
• Ability to read, write, speak and comprehend the English Language

• Must maintain a professional appearance and professional communication behaviors with clients and family members/caregivers.
• Ability to competently identify changes in client condition and effectively communicate changes to family members/caregivers.
