Job summary

Community LIFE is seeking an experienced Utilization Management Coordinator to manage and coordinate utilization across care levels. The role ensures appropriate service authorization in line with policies and clinical criteria.

The candidate works with RN Case Managers to align goals of care and collaborates across settings, including skilled nursing facilities, home care, and outpatient services. PA licensure is required and a valid driver’s license is needed.

Qualifications

• RN/PA licensure or PT/OT/OTA licensure required in PA
• Bachelor or higher related degree as listed in job ad
• Licensure must be active and in good standing in Pennsylvania
• Experience in utilization review or case management preferred

Responsibilities

• Coordinate utilization management across levels of care
• Support hospital and nursing facility service authorization processes
• Collaborate with RN Case Managers to align care goals
• Review clinical information to determine medical necessity and appropriate levels of care
• Coordinate discharge planning and post-acute care needs
