Job summary

Vitae Health Systems is hiring a Care Transitions Utilization Nurse (LPN) to manage patient needs during post-acute transitions. The role partners with embedded NPs to coordinate discharge planning and telephonic follow-up for residents transferring to ED, hospital, or community settings.

The role is hybrid with one day per week in the Park Ridge, IL office, reporting to the Executive Director of Value-Based Care Clinical Operations.

Qualifications

• Active, unrestricted LPN license in Illinois (or compact eligibility).
• 5+ years in skilled nursing facility leadership or advanced role.
• Strong SBAR communication and clinical judgment.

Responsibilities

• Identify ED visits/hospital admissions and document details.
• Review events with NP discussion and escalation when needed.
• Conduct post-discharge telephone outreach to ensure follow-up care.
