Job summary

Enhabit Home Health & Hospice is seeking a Care Transitions Coordinator (Clinical Liaison) to guide patients through post-acute care and strengthen relationships with health systems, hospitals and physicians. You will implement evidence-based guidelines to create effective transition plans focused on quality and efficiency.

The role requires RN/therapist/social work background, state licensure, and at least two years of field experience, with mobility and tech proficiency essential for success.

Qualifications

• Must have basic demonstrated technology skills, including operation of a mobile device.
• Must be a graduate of an approved school of nursing, therapy or social work.
• Must be licensed in the state where they currently practice.
• Must have two (2) years of demonstrated field experience.
• Must understand federal, state, and local laws and regulatory guidelines governing Medicare certified home health and hospice.

Responsibilities

• Assist patients with navigating post-acute care and improve patient outcomes while increasing referral source satisfaction with Enhabit's services.
• Integrate evidence-based clinical guidelines, preventative guidelines, protocols, and other metrics in transition plans that are patient-centered and promote quality and efficiency in post-acute care.
