Job summary

Duke Regional Hospital in Durham, North Carolina, seeks a skilled Case Manager to oversee discharge planning, care transitions, and coordination across the continuum of care to support optimal patient outcomes.

Responsibilities include assessing needs, developing discharge plans, coordinating services, and educating patients and families. Requirements include BSN or MSW, 3 years' experience, and CM certification within 3 years.

Qualifications

• BSN or MSW required.
• Minimum of 3 years of relevant experience.
• Case Management Certification (ACM/CCM/ANCC) within 3 years of hire.
• RN license (or compact) in NC if BSN-prepared.

Responsibilities

• Assess patients for case management needs, including discharge planning and care transitions.
• Develop, implement, and evaluate individualized discharge plans.
• Coordinate services such as Home Health, DME, transportation, SNF, and post-acute referrals.
• Monitor daily census and evaluate discharge needs.
• Communicate findings and orders with the healthcare team.
• Educate patients and families on benefits, reimbursement guidelines, and regulatory requirements.
• Collaborate with Utilization Management to support payer negotiations.
