Job summary

• Associates degree required.

Responsibilities

• Assess potential residents/guests and determine admission in collaboration with physicians and nursing staff.
• Develop and maintain relationships with discharge planners, physicians, families, senior living providers, and home health agencies.
• Educate referral sources about program services and available resources.
• Coordinate with staff to ensure a smooth discharge process.
• Actively communicate with prospective residents, families, and influencers to generate activity.
• Monitor market changes and make recommendations with action plans.
• Support occupancy targets in line with the marketing plan.
• Transmits information to the Admissions Coordinator.
