Job summary

Texas Health Frisco is seeking a Care Transition Specialist in Case Management to coordinate patient transitions and ensure seamless care. This on-site role supports caregivers and operates in a fast-paced hospital setting in Frisco, TX.

You will handle clerical processes, manage communication with providers, and assist with transition planning under care transition managers, requiring a High School diploma and 3 years of relevant experience; strong patient care or customer service is preferred.

Qualifications

• H.S. Diploma or Equivalent required
• Three years’ relevant experience, preferably in a healthcare or service environment required
• Patient Care/ Customer Service / Healthcare experience strongly preferred

Responsibilities

• Coordinate the clerical and administrative processes for entity care transitions management
• Manage deparmental mail box, phone lines and/or fax
• Support transition planning activities as needed under the direction of care transition managers
• Calls vendors, physician offices, clinics, etc
• Faxes (electronic and traditional) documents as needed
• Makes copies or gathers other data as needed
• Collect and enter data as needed
• Completes other tasks and projects assigned
