Job summary

Texas Health Resources in Fort Worth is seeking a Care Transition Manager RN for a PRN role. You will support discharge planning, care coordination, and interdisciplinary collaboration within the Care Transition Management Department.

The role requires a BSN, 3 years of staff nurse experience in an acute care hospital, CPR certification, and preferred ACM/ CCM or ANCC credentials. Training is 5 weeks; hours are mainly weekdays; work location is at Texas Health Ft.

Qualifications

• Bachelor’s degree in Nursing (BSN) required.
• RN - Registered Nurse upon hire required.
• CPR - Cardiopulmonary Resuscitation upon hire required.
• 1 year discharge planning/care management highly preferred.
• ACM - Accredited Case Manager upon hire preferred Or CCM - Certified Case Manager upon hire preferred Or ANCC upon hire preferred.

Responsibilities

• Working knowledge of medical necessity criteria preferred.
• Identify community resources and service needs and facilitate appropriate referrals as needed.
• Ability to engage in complex clinical decision-making and discharge planning.
• Strong oral and written communication skills.
• Ability to prioritize and meet deadlines.
