Job summary

Texas Health Resources in Flower Mound, TX is seeking a Care Transition Manager, RN. The role requires a Registered Nurse with a Bachelor’s degree and strong discharge planning and care coordination skills.

You will work in a full-time daytime schedule with occasional weekend coverage. The ideal candidate has 3 years as a staff nurse in acute care, 1 year in discharge planning or RN case management, CPR certification, and holds or pursues ACM/CCM credentials.

Qualifications

• Bachelor's Degree in Nursing required.
• 3 Years Staff Nurse at an acute care hospital required.
• 1 Year discharge planning/care coordination preferred.
• 1 Year hospital RN Case Management experience preferred.
• Registered Nurse (RN) license required upon hire.
• Current CPR required upon hire.
• ACM certification preferred.
• CCM certification preferred.
• ANCC certifications preferred.

Responsibilities

• Completes Transition Evaluations and collects Social Determinants of Health (SDOH) data within 48 hours of identification and begins discharge planning.
• Participates in multidisciplinary rounds to identify length of stay, expected discharge date, and barriers to discharge.
• Assists in determining level and type of care needed and coordinates care.
• Schedule/coordinate patient clinical needs to post-acute care facilities based on capabilities and patient choice.
• Educates payors and multidisciplinary team regarding payor requirements and coordinates care with payors.
