Job summary

Methodist Dallas Medical Center is seeking a Care Transitions Navigator to coordinate activities that promote quality outcomes, patient throughput and discharge planning while supporting optimal care and resource utilization.

The role focuses on identifying barriers to throughput and minimizing discharge delays, with education requirements including a BA/BS in Social Work and SW licensure preferences.

Qualifications

• Bachelor’s degree in Social Work required.
• Master’s degree in Social Work preferred.
• LMSW/LBSW, or RN as licensed by the Texas Board of Examiners, CCM or ACM preferred.

Job Purpose

• The Care Transitions Navigator will coordinate activities that promote quality outcomes, patient throughput and discharge planning while supporting a balance of optimal care and appropriate resource utilization. The Care Transitions Navigator will identify potential barriers to patient throughput and quality outcomes minimizing delays in discharge plans.

Job Requirements:

Education:

• Bachelor’s degree in Social Work required
• Master’s degree in Social Work preferred

Licenses and/or Certifications:

• LMSW/LBSW, or RN as licensed by the Texas Board of Examiners, CCM or ACM preferred

Work Experience:

• 1 year of experience in health related setting
• Hospital case management experience preferred
