Job summary

Adoration Health is looking for a Care Transition Coordinator in Little Rock, Arkansas. This role involves facilitating patient transitions from healthcare facilities to home health or hospice care, coordinating care plans, and acting as a liaison between the agency and healthcare providers.

The ideal candidate will have a strong background in home health case management, excellent communication skills, and a current nursing or social work license. Experience in medical marketing is preferred.

Qualifications

• Minimum of one (1) year of experience in home health or hospital‑based case management.
• One (1) to three (3) years of experience in medical marketing or healthcare business development (preferred).
• Current and active licensure in the state of practice as a Registered Nurse, LPN, SW, or PT is required.

Responsibilities

• Achieve monthly personal production goals for assigned locations.
• Evaluate patients for home care eligibility according to guidelines.
• Coordinate transfer orders and educate patients on home care services.
